Take a paper cup, fill it with water, put a popcicle stick in the cup.
Put in the freezer. When frozen, pull/tear paper cup away from ice.
    Apply to desired area.

Yep, Bombero gave a nice home-made alternative. The real deal is a 2-piece thick plastic cup that separates to provide a nice base to hold onto for ice massage (I managed to score one from a PT friend of mine--really nice).

Also, you could hold 1-2 ice cubes (depends on size of cubes) with a wash cloth so they kind of stick together (the wash cloth to protect your fingers) and massage. You're only looking for 3-5 minutes, just enough to numb the area. So by the time the ice cubes melt enough that you can't hold them, you're done.

    Hope this helps!

You don't even need the popsicle stick. Just hold the base of the cup and keep peeling down the top part of the cup as it melts during application.

During the my track season my coach would bring a cooler filled with frozen water in regular paper cups. Just peel away the paper as it melts. Good stuff.
